BRIDGES & PPL: Strengthening CDPAP for You

BRIDGES has been approved as a facilitator for the Consumer Directed Personal Assistance Program (CDPAP).

We will be partnering with the new statewide fiscal intermediary, Public Partnerships LLC (PPL), to make sure you continue receiving the best care from your trusted personal assistants. 

What This Means for You

  • -Your eligibility for CDPAP and the services you receive will stay the same. 
  • -Services to consumers and payments to personal assistants will continue without interruption. 
  • -We will make sure you continue receiving care and services in the ways that matter to you.

What You Need to Do Now

You will need to transition to PPL by March 28, 2025. You can work with BRIDGES to complete the process. 

BRIDGES will reach out to you to start the transition and make sure you are all set to continue with CDPAP. However, you do not have to wait to hear from us. Starting January 6, 2025, you can call us at (845) 624-1366 ext. 152 or email us at Cdpa@bridgesrc.org

Here are the steps to transition:
Consumers
  • - Connect with BRIDGES
  • - Complete the necessary forms (online through PPL@Home system or in-person at a BRIDGES office)
  • - Have your PAs complete the necessary forms and training
Personal Assistants
  • - Work with your consumer to initiate the transition
  • - Fill out, sign, and submit the necessary forms and documents (online through the PPL@Home system or in-person at a BRIDGES office)
  • - Complete a health assessment (only if expired)
  • - Complete the necessary PPL trainings

BRIDGES CDPA Program improves quality of life for people with disabilities and older adults who require personal assistance at home. It allows those in need of personal assistance to hire their own caregivers and direct their own care.  CDPA gives people independence within the community.
